Other documents that are required to complete the registration process:

• Student's birth certificate

• Student's immunization records

• Proof of residence (a piece of mail or utility bill with your physical address on it)

• Hearing/Vision/Dental Screening
  The State of Michigan law requires that all students entering Young 5’s or Kindergarten must provide proof of receiving a vision, hearing, and dental screening within the last year. Documentation must be provided to Houghton Elementary school office prior to the first day of class in September.
  
  Hearing and vision screenings can be obtained through your family physician, optometrist, or audiologist. The Western UP District Health Department can also perform hearing and vision screenings for free. Please call the Health Department at 906-482-7382 ext. 171 to schedule an appointment.
  
  Dental screenings can be obtained through your dentist’s office by either completing the attached Oral Assessment form or by a simple letter from the dentist’s office stating the date of the student’s last appt.
