Registration Form

Dear Parents,
Attached you will find our Back to School Registration form. This form is very important and we are asking HPTS families to complete this by noon on Monday, August 17.

The registration form will ask you to select a learning option for each child in your household. We will need to begin enrolling students who select the virtual option and scheduling students in a face to face schedule. There are also two questions regarding transportation and busing.

Please know that you can email building principals with questions regarding the face to face or virtual options. I know this is a difficult decision, but we are committed to keeping you informed and helping all students succeed. Thank you for your support.
